Faith Lutheran believes that a child’s caretakers are called to be their primary faith teachers. Our responsibility as church staff is to support caretakers as they raise their children in the knowledge and love of Christ. Because of the promises made at Baptism, and the Biblical command, we expect that caretakers will be an active part of the Confirmation process. The checklist below will provide students and caretakers with an organized method in which to keep all confirmation requirements in one place. All worship notes, service hours, and mentor session records should be documented on this form (or on the online form).

Worship notes:9 Sundays and 3 Lenten Services = 12 total

Use this space as a checklist for completed Worship Notes. Students will still need to fill out a “Worship Note” form and place it in their folder or submit it online. At least 8 must be completed at Faith Lutheran; up to 4 can be from another church. Wednesday evening Prime Time worship services are counted as “Sunday” worship notes. All Holy Week services (except Easter) can be used as Lenten services, but we ask that students attend at least one Wednesday night Lenten service.
